First Day Forms

These ten documents are required before your child’s first day of school at The Odyssey Montessori School.

        1. Identification and Emergency Information 

2. Consent for Emergency Medical Treatment 

3. Child’s Preadmission Health History

4. Personal Rights

5. Notification of Parents’ Rights

6. Physician’s Report

If the Physician’s Report is not ready for your child’s first day, you can submit it within the first month, but you must bring a vaccinations record the first day.
